Please read carefully. I agree, acknowledge and declare that:

I am authorized to enter into the Agreement, formed by this Booking Form and the terms and conditions (which I have read, understood and agree to);

Tax Invoice and Tax Receipt. This is a Booking Form and self generated Tax Invoice (for the items checked above) from HPIQ (ABN 30 134 798 502). When paid, this Tax Invoice becomes a Tax Receipt. Print this completed form and retain your records now before hitting "Send Pool Inspection Request". No other Tax Invoice will be issued.

To the maximum extent permitted by law, HPIQ's liability (including that of its directors, officers, employees and contractors) to me under this agreement is limited to the amount paid by me to HPIQ;

Understand that occasionally, additional charges may apply. For example, Pool Safety Re -Inspection $105 plus $50/hour travel (if more than 20 minutes). This only applies if HPIQ cannot repair the pool fence to comply with the Act during the first visit.

HPIQ discounts its fees (by about 15-20%) if I book online and pay into the HPIQ bank account before HPIQ attends at the Pool for the first time. The HPIQ bank account details are:

Account number: 45-265199-8
Reference: When making payment, please include in the ‘pay to’ reference box:
• your house number; and
• your street name.
For example if you live at 37 Bronco Street, Kenmore, Brisbane 4069 then your reference is “37 Bronco”. Please email your payment acknowledgment to accounts@HPIQ.com.au.

I will pay HPIQ's fees before HPIQ attends at the Pool and if I do not do that then I will pay the full (not discounted) fees.

If I do not pay HPIQ's fee in full within 30 days of HPIQ attending at the Pool for the first time then I agree to pay a late payment fee of $50 for the first reminder and then 2%/month thereafter plus all recovery costs (including collection, court filing and legal fees).
